Requirements Checklist

  1. Must be Clayton County resident

  2. Must be negatively affected directly or indirectly by COVID-19

  3. Must complete entire assessment process beginning with an intake form and uploading of all requested documents

Required Documents

Please bring all of these documents if you have made an in-office appointment.

  1. Proof of income

  2. Proof of unemployment filing (if applicable)

  3. Copy of State ID for everyone 18 years old and over in your household

  4. Proof of COVID-19 related hardship

  5. Copy of mortgage statement (if applicable)

  6. Full Lease agreement

  7. Late notice, letter from landlord or mortgage company stating the current-full amount due.

    • Must have detailed breakdown of all charges and dated within five days of your application.

  8. Social Security Card (Everyone aged 6+)

  9. Court Documents - Eviction Notice, Dispossessory, Writ etc.
